Risk thresholds

< 10%: Low risk
10–20%: Intermediate risk
> 20%: High risk — consider statin therapy equivalent to secondary prevention

Clinical pearls

ATP III guidelines used this score to guide LDL targets and statin initiation.
2013 ACC/AHA shifted to the Pooled Cohort Equations (ASCVD risk), but Framingham remains widely recognized.
Risk-enhancing factors (family history, CRP, CAC score, ankle-brachial index) can refine risk estimates.
